Generalized edema and hyperdynamic circulation. A possible case of beriberi.

چکیده
176 vascular web (fig. 1). The electrocardiogram showed sinus tachycardia and incomplete right bundle-branch block (fig. 2). The echocardiogram on hospital admission showed enlargement of the left chambers, in addition to high values of shortening fraction and ejection fraction (tab. I). The results of the complementary tests in addition to the clinical findings were compatible with a hyperdynamic status of circulation. Because the patient had no anemia, hyperthyroidism, or evidence of arteriovenous fistula, the diagnostic hypothesis of beriberi was established. Three days after the introduction of thiamine, the patient’s edema improved, the cardiac murmur disappeared, and, on chest X-rays, the cardiac area decreased (fig. 3). The echocardiography performed 7 days after the introduction of thiamine was normal (tab. I). The patient has been followed up on an outpatient basis, currently being asymptomatic.

Beriberi heart in Iowa veterans.
Beriberi heart disease does not constitute a rigid syndrome because the severity of cardiac failure, associated leriplleral neuritis, other concomitant signs and syimptomiis, and response to treatment are variable. Five lpatiellts with peripheral neuritis, edema and evidence of cardiac failure are presented. All improved after treatment, and, in two instances, there was marked reduction in card...
متن کاملCase report: pericardial effusion with constrictive physiology in a patient with wet beriberi
Wet beriberi-induced pericardial effusion has rarely been previously described. Little is known about the effect of beriberi-induced pericardial effusion on hemodynamics. Here we present a case of wet beriberi with pericardial effusion that exhibited constrictive physiology, which was dramatically improved after treatment. A 61-year-old male patient was admitted to our hospital for progressive ...
متن کاملThiamin deficiency as a cause of reversible cor pulmonale.
Thiamine deficiency may present four classic clinical forms: peripheral polyneuropathy, anorexia and muscular weakness (dry beriberi); high output heart failure with signs of congestion (wet beriberi); beriberi associated with shock (Shoshin beriberi) and Wernicke's encephalopathy. In this report we describe a picture that is suggestive of severe pulmonary hypertension and cor pulmonale, with j...
متن کاملA Case of Beriberi with Leg Edema, Pleural Effusion, and Anemia
Case: We describe a case of beriberi with leg edema, pleural effusion, and anemia. Echocardiography revealed that the inferior vena cava diameter was 18 mm, but respiratory changes were not observed. Pulmonary hypertension was observed, with a systolic pulmonary arterial pressure of 45 mmHg. Peripheral blood concentrations of vitamin B1 decreased significantly. Beriberi heart disease caused by ...
متن کاملClinical History and Colliquative Myocytolysis Are Keys to the Diagnosis of Shoshin Beriberi
Cardiovascular beriberi presents as either the fulminant (Shoshin beriberi) or chronic form. Shoshin beriberi is a rare disease that may lead to a fatal outcome if the patient does not receive appropriate treatment. In the present report, we describe the case of a 66-year-old man presenting with leg edema and dyspnea at rest. Clinical presentations were nonalcoholic Shoshin beriberi and lactate...
